Responsibilities
- Provide ongoing support, guidance and role modeling for individuals.
- Assist individuals in daily living activities, promoting self-advocacy, decision-making, problem-solving, and empowerment.
- Collaborate with the individual, family members, and program management to provide support.
- Plan and implement activities and daily routines for individuals.
- Encourage individuals to engage in community activities and seek out opportunities for integration.
- Support individuals in setting and achieving person-centered goals and plans.
- Lift and transfer individuals as needed.
- Prepare summaries of interactions and behavioral observations.
- Provide transportation as needed.
- Attend and actively participate in all required trainings; maintain necessary certifications.
- Report significant medical, behavioral, and psychiatric health-related concerns.
- Perform physical intervention in case of a client crisis.
Qualifications
- A High School Diploma or GED.
- Experience with the program population is preferred.
- A high energy level and ability to work in a team environment.
- Effective verbal and written communication skills, and the ability to make sound judgments.
- Ability to work independently and as part of a team.
- A valid driver's license, operational and insured vehicle, and willingness to transport clients.
- A demonstrated understanding and competence in serving culturally diverse populations.
