About the role
The role of Pediatric Home Care Specialist & Acute Care Registered Nurse at Imagine Pediatrics involves delivering high-quality, family-centered care in the home and community, collaborating with an integrated care team, conducting in-home assessments, performing procedures, providing patient and family education, and flexing into the virtual acute care RN role to manage urgent pediatric needs.
Responsibilities
- Travel to members' homes and community settings to deliver episodic, longitudinal, and urgent pediatric care.
- Build trusted, compassionate relationships with children and families, ensuring coordinated, culturally responsive care.
- Collaborate within an integrated care team including physicians, APPs, nurses, pharmacists, dietitians, navigators, social workers, and therapists.
- Conduct and document in-home assessments; perform procedures such as ear irrigation, point-of-care testing, IV placement and lab draws, wound care, and medication administration.
- Provide patient and family education, follow-up care, and care coordination using the Imagine Pediatrics platform.
- Flex into the virtual acute care RN role to conduct virtual assessments and manage urgent pediatric needs using established protocols.
- Document care in the EMR (Athena) and communicate with families through Imagine Pediatrics’ proprietary application.
- Utilize virtual platforms and digital tools confidently to support care delivery.
Requirements
- Active TN RN license required, and compact license preferred.
- Bachelor's degree in nursing (BSN) is required.
- EMS (EMT/P) experience and 2+ years of pediatric home care experience preferred.
- Bilingual Spanish is preferred.
- Minimum 3+ years of pediatric nursing experience, ideally including high-acuity settings (ED, urgent care, transport, or similar).
- 1–2 years in pediatric emergency medicine experience.
- Valid driver's license, reliable transportation, and good driving record.
- Comfort driving long distances in varied weather conditions.
- Strong clinical judgment, communication skills, and ability to work independently in the home environment.
- Comfort with EMRs, virtual platforms, and mobile documentation tools.

- Current residence in or around Nashville area.
- PALS & BLS required.
