Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Multi-state licenses preferred
• Able to cover specified primary locations

Pay and Benefits

• Hourly base rate, plus additional incentive structure
• Malpractice insurance coverage (during Signify Health-related activities)

• Eligible for other reimbursement opportunities, including pay per completed Diagnostic and Preventive Services (DPS)
