In-Home Physician Full Time - Rockford

Responsibilities

• Engage with patients in their homes to conduct health evaluations
• Review medication and medical history with patients
• Perform physical evaluations and additional diagnostic tests as needed
• Provide personalized care connections based on patients' unique needs
• Document findings using an iPad and follow an intuitive clinical workflow
• Conduct virtual visits when necessary
• Travel locally and within licensed states as required

Qualifications

• Active, unrestricted license in coverage areas
• Multi-state licenses preferred
• Experience with in-home health evaluations
• Strong interpersonal and communication skills
• Ability to work independently and manage time effectively
• Comfortable working in diverse home environments
