In-Home Physician Full Time - Cincinnati, OH

Responsibilities

• Conduct personalized in-home health evaluations for individuals
• Build trusted relationships with patients to improve health outcomes
• Provide comprehensive assessments including medication and medical history reviews
• Perform physical evaluations and additional diagnostic tests as ordered
• Complete virtual visits when necessary
• Engage with various health plans to bridge gaps in care
• Document findings and recommendations using an iPad and clinical workflow

Qualifications

• Active, unrestricted license(s) in coverage area(s) required
• Multi-state licenses preferred
• Experience in patient care with a focus on in-home evaluations
• Excellent communication and interpersonal skills
• Ability to work independently and travel locally as needed
• Familiarity with technology for clinical documentation
