In-Home Physician

About the Role

Conduct comprehensive health assessments for patients in their homes, review medical histories and medications, identify care gaps, and document findings using our technology platform. Work with care teams to develop personalized care plans, provide patient education, and recommend follow-up actions in a mission-driven environment focused on value-based care and improving outcomes for vulnerable populations.

Requirements

• Medical degree
• Licensed physician
• Strong clinical evaluation skills
• Ability to interpret health data
