In-Home Physician

Responsibilities

• Perform comprehensive in-home evaluations for Medicare Advantage and vulnerable populations
• Conduct detailed medical histories, physical exams, and risk assessments
• Update medication lists and identify care gaps to support value-based care
• Utilize EHR and clinical decision tools to document findings
• Collaborate with interdisciplinary teams to improve patient outcomes
• Deliver patient-centered evaluations at home, leverage technology for documentation, and support care coordination to enhance healthcare delivery for vulnerable populations

Qualifications & Skills

• Medical degree and valid physician license
• Experience in primary care, geriatrics, or home-based care preferred
• Strong clinical assessment and documentation skills
• Proficiency with electronic health records and clinical tools
• Excellent communication and teamwork abilities
