About the role

As an In Home Patient Care Assistant (PCA), you will provide exceptional care to elderly, vulnerable, disabled, and dependent clients, prioritizing their needs above all else.
Responsibilities
- Assist the client with Activities of Daily Living (ADLs) as specified in the client’s plan of care, including but not limited to:
- Bathing, dressing, grooming, and other personal hygiene
- Toileting, transferring, eating, and continence care
- Provide medication assistance, housekeeping services, laundry, meal preparation, feeding, shopping, and transportation to and from appointments
- Offer companionship and perform other household tasks as specified in the client’s plan of care
- Report daily activity to your supervisor through the agency management system, daily activity logs, and other communication avenues (phone calls, emails, agency app, face-to-face)
- Practice safety in all activities concerning Avenues Home Care clients
- Lift or push up to 50 pounds
- Demonstrate honesty, accountability, and integrity
- Provide superior care and remain vigilant of any potential care concerns or changes in condition
- Maintain personal appearance in compliance with Avenues Home Care policies, including dress code
- Be timely in reporting for shifts or making scheduling changes
- Report any suspected abuse or neglect immediately up the proper chain of command, as required by law
- Document any unusual behavior, events, conditions, or people
- Maintain confidentiality of records, correspondence, and other sensitive information
- Attend meetings and in-services as requested
- Abide by all policies and procedures set forth by the company
- Ensure protection of confidential information in compliance with company policies and HIPAA regulations
- Perform other duties as assigned
Requirements
- 18 years of age or older
- Valid driver’s license, automobile insurance, and reliable transportation
- Ability to lift or push up to 50 pounds
- Good communication skills and ability to follow verbal and written assignments
- Proficiency with Homecare Company Management System (ClearCare)
- Maintain an established work schedule
- Effective interpersonal, communication, organizational, and planning skills
- Ability to establish and maintain effective working relationships
- Maintain confidentiality of work-related information and materials
- Maintain “Fit for Duty” physical condition
Preferred Qualifications
- CNA License or alternate experience as a caregiver
- CPR & First Aid Certifications
Skills
- Interpersonal and communication skills, including tact and diplomacy
- Organizational and planning skills with attention to detail and follow-through
- Ability to maintain confidentiality and comply with HIPAA regulations
