In Home Nurse Supervisor

About the Role

Phoenix Home Care & Hospice is looking for an experienced RN Nurse Supervisor to support our Advanced Personal Care clients and nursing team in the Independence, MO area. This full-time, day-schedule position focuses on mentoring nurses, building strong relationships, and providing steady, supportive leadership in home-based care.

Responsibilities

• Complete initial and reassessment visits for Advanced Personal Care clients
• Build and maintain strong relationships with staff, patients, families, and caseworkers
• Provide training, certification support, and ongoing education for nursing staff
• Supervise, coach, and evaluate nurses in the field
• Ensure services meet company standards and state regulations
• Serve as a clinical resource and support problem solving in real time
• Maintain accurate and timely electronic documentation
• Participate in quality initiatives, meetings, and team development
• Assist with hiring and onboarding as needed
• Support the team wherever needed to ensure quality care delivery

Requirements

• Active RN license required
• Graduate of an approved nursing program
• At least 1 year of professional nursing experience within the last 3 years
• Supervisory experience preferred
• Home health experience preferred
• Valid driver’s license and insured vehicle
• Ability to pass background check, drug screen, and physical
