In-Home Nurse Practitioner Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-time Nurse Practitioner to provide In-Home Health Evaluations. You’ll engage with people in the comfort of their own homes, helping bridge gaps in care by building trusted relationships and delivering flexible, comprehensive, and personalized health evaluations.
The visit lasts about 45 minutes and includes a one-on-one connection, medication and medical history review, a physical evaluation, and—if ordered by the person’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.
Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests as ordered by health plans.
- Build trusted relationships with health plan members to improve health outcomes.
- Travel locally and within licensed states to perform in-home visits.
- Conduct virtual visits as needed.
- Provide Signify Health services, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams).
Work Environment
As an in-home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present varying conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be comfortable and safe performing duties in such settings.
Qualifications
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
Schedule
- Part-time: 29.5 anticipated weekly hours.
- Flexible travel required within licensed states.
Pay
The typical pay range for this role is $37.66 - $81.11 per hour. The actual base salary offer will depend on factors such as experience, education, and geography.
