In-Home Nurse Practitioner or Physician Assistant Part Time

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
Responsibilities
- Conduct 45‑minute in‑home health evaluations, including medication and medical history review, physical evaluation, and ordered diagnostic tests (e.g., diabetic eye exams, spirometry).
- Use an iPad and intuitive clinical workflow for each evaluation.
- Build trusted relationships with health plan members and help bridge gaps in care.
- Travel locally and within licensed states as needed; conduct virtual visits when required.
- Provide all Signify Health services, including Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).
- Do not prescribe medicine, order lab tests, or alter current treatment regimens.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Multi‑state licenses preferred.

Pay
The typical pay range for this role is $37.66 – $81.11 per hour (base hourly rate). Actual offer depends on experience, education, geography, and other factors.
Schedule
Anticipated weekly hours: 29.5 (part‑time).
