You will partner with cross-functional stakeholders to clarify priorities, solve operational challenges, and move important work forward, contributing to meaningful outcomes while strengthening systems within a healthcare environment.

Responsibilities

• Conduct in-home health evaluations and communicate effectively during visits.
• Build trusting relationships with patients to improve health outcomes.
• Provide diagnostic and preventive services as needed, such as spirometry and bone density screenings.
• Travel locally within licensed states to complete health evaluations.
• Conduct virtual visits to enhance patient accessibility.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Experience in providing in-home evaluations or similar settings.
• Strong interpersonal skills for patient engagement.

Pay

$95,738 – $206,206 annually.
