About the role

Responsibilities
- Administer feeding tube and medication administration as per physician orders
- Provide care to patients in their homes, ensuring their comfort and safety
- Document patient care in an electronic format using tablets provided in each patient's home
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Adhere to infection control protocols and maintain a clean, safe environment
Requirements
- Maintain an active, unencumbered license (LPN/LVN) in the state of practice
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation
Qualifications
- Must have a valid driver’s license
- Must pass a background check
- Must adhere to all healthcare regulations and standards
