In-Home Health - Physician (Per Diem) - Providence, RI

Signify Health, part of CVS Health®, is hiring per diem physicians (MD/DO) to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. These clinicians will work with us during peak demand months and/or in high-demand areas, depending on business needs.

Responsibilities

• Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area (primary coverage areas include Cambridge, MA and surrounding counties).
• Provide availability to complete in-home visits in areas of business need and/or during seasonal spikes in demand for visits.
• Execute a strong visit completion rate.
• Provide availability Monday through Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m.

Requirements

• Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s).
• Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics.
• Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required).
• Maintain all required supplies and associated diagnostic equipment related to DPS offerings.
• Maintain active medical licenses (MD/DO) in coverage area(s).
• Multi-state licenses (preferred).
• Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred).
• Be flexible to travel locally and within licensed states, as needed (up to 25%).
• Willingness to execute a collaborating physician agreement, partnering with and overseeing Signify Health-contracted nurse practitioners or physician assistants and fulfilling all state-by-state requirements (preferred).
• Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies, and scale).
• Accommodate field observations and/or “ridealong” experiences, as requested.
