In-Home Health - Nurse Practitioner (Per Diem - Michigan City)

About the Role

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner) to conduct In-Home Health Evaluations. This role focuses on engaging with individuals in their homes to bridge gaps in care, build trusted relationships, and deliver personalized health evaluations.

The visit, typically lasting about 45 minutes, includes a one-on-one connection, medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter existing treatment regimens.

Responsibilities

• Bring empathy and care into every visit as part of a national network of clinicians dedicated to improving health outcomes.
• Conduct in-home health evaluations for millions of people nationwide, connecting them to the right care for their unique needs.
• Travel locally and within licensed states as needed, with flexibility to conduct virtual visits.
• Provide Signify Health services, including Diagnostic & Preventive Services (e.g., spirometry, diabetic eye exams, bone density screening).

Work Environment

As an in-home clinician, your primary workplace is the private residence of health plan members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.

Requirements & Qualifications

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Pay

The typical pay range for this role is $37.66 - $81.11 per hour. The actual base hourly rate will depend on factors such as experience, education, and geography.
