This full-time position focuses on delivering in-home health evaluations, building patient relationships, and improving health outcomes through clinical assessments and technology-enabled workflows.

Responsibilities

• Conduct in-home health evaluations to assess patient health
• Engage with patients to build trusted relationships and improve health outcomes
• Perform medication and medical history reviews
• Utilize technology (iPad) for clinical workflow and documentation
• Offer diagnostic services such as spirometry and bone density screening
• Travel locally and within licensed states as necessary
• Conduct virtual visits when needed

Requirements

• Active, unrestricted license(s) in coverage area(s) (Seattle/King)
• Board certification required
• Multi-state licenses preferred
