Responsibilities
- Performs or assists patient with personal care activities as prescribed in the plan of care, including but not limited to hygiene, mobility and elimination. If unable to perform a certain task, reports to the RN or Director of Clinical Services immediately.
- Provides and maintains a safe environment for patient. Uses equipment safely and properly (footstools, side rails, Oxygen, etc.).
- P Practices Universal Infection Control Precautions in all contacts with patients.
- Performs or assists patient/family with home maintenance activities as prescribed in the care plan, including but not limited to light housekeeping and meal preparation.
- M Monitors adequate fluid and nutrition intake, temperature, pulse, respiration, skin and basic elements of body functioning and changes in body functioning and record/report appropriately.
- P Practices safe transfer, ambulation, normal range of motion and positioning techniques.
- R Respects the autonomy and right to confidentiality of all patients and families.
- D Document scare provided and response of consumer/family. Submits documentation in a timely manner.
- P Performs within the limits of preparation and experience.
- E Ensures compliance with local, state and federal laws, Medicare regulations and established agency policies and procedures.
- M Manages a caseload and delivers care in a cost-effective manner.
- C Consistently meets or exceeds 365 Health Services’ established service standards.
- C Conducts all business activities in a professional and ethical manner.
- U Uses supplies and equipment effectively and efficiently.
- C Completes a minimum of 12 hours of in-services per year.
- M Maintains regular attendance.
- A Adheres to 365 Health Services’ Code of Conduct.
- I Interacts with all staff in a positive fashion supporting 365 Health Services’ mission and vision.
- C Conducts all business activities in a professional and ethical manner.
- C Complies with 365 Health Services’ general orientation and with agency policies and procedures including confidentiality and HIPAA guidelines.
Qualifications
Required:
- High school diploma or equivalent.
- Valid Pennsylvania Driver’s License.
- Current CPR certification.
- Experience in home health care or related field preferred.
Skills
- Strong communication skills.
- Ability to work independently and as part of a team.
- Knowledge of infection control practices.
- Basic first aid and emergency response skills.
- Ability to lift and move patients as needed.
