In-Home Caregiver

About the role

Welcome to WelbeHealth PACE, where we support seniors in maintaining their independence and quality of life through comprehensive medical care and community-based services. Our Home Care Assistants play a crucial role in our team, providing essential support to our participants while also contributing to the care planning process.

Responsibilities

• Provide safe delivery of personal care services to WelbeHealth participants in their homes, as scheduled and directed.
• Work a stable, full-time schedule, visiting 2-5 patients per day depending on participant needs and care plans.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes.
• Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan.
• Provide translation services as needed.

Qualifications

• High school diploma or equivalency.
• Current Basic Life Support (BLS)/First Aid Certification.
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
• Bilingual preferred.
• Registered with Home Care Registry prior to start date.
• Obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

$22—$25 USD
