About the role
We are seeking a compassionate and motivated medical provider to join our team. In this role, you will manage a patient panel of up to 500 individuals, blending membership-based care with Medicare services to deliver personalized, patient-centered care. You’ll collaborate closely with other medical providers, nurses, and administrative staff to emphasize continuity, prevention, and holistic well-being for each patient.
Key Responsibilities
- Deliver comprehensive, evidence-based care to patients of all ages within our membership and Medicare hybrid model, ensuring each patient receives personalized attention and support tailored to their individual health needs.
- Collaborate with a multidisciplinary team of providers, nurses, and administrative staff to create personalized care plans that foster seamless care coordination and uphold the clinic’s commitment to high-quality, patient-centered care.
- Perform thorough patient evaluations, including history-taking, physical exams, diagnostic testing, and customized treatment plans, ensuring care is aligned with each patient’s unique health journey.
- Utilize remote patient monitoring and innovative technologies to enhance chronic disease management, improve patient engagement, and promote proactive, preventative care that supports long-term health and wellness.
- Participate in care team meetings, ensuring a holistic, integrated approach to patient care that aligns with the clinic’s mission of continuity, prevention, and well-being.
- Provide home or facility visits as needed, ensuring that patients receive flexible, accessible care and have additional support when necessary (note: non-Medicare patients incur an additional charge).
- Maintain accurate, detailed medical records that comply with clinic protocols and regulatory standards, ensuring patient information is up-to-date and easily accessible.
- Engage in quality improvement initiatives to enhance clinic operations and patient outcomes, ensuring the clinic’s continued alignment with its mission to provide accessible, compassionate care for our community.
Qualifications
- Valid licensure as a Nurse Practitioner, Physician Assistant, or Physician in the state of Washington.
- Board certification in Family Medicine, Internal Medicine, or a related specialty.
- A strong commitment to team-based care, with a focus on delivering personalized care to a diverse patient panel.
- Experience or strong interest in managing Medicare patients and a solid understanding of value-based care models.
- Proficiency with electronic health records (EHRs) and remote monitoring tools, with a focus on enhancing patient engagement and improving chronic care management.
- Excellent communication and interpersonal skills, with a patient-first mindset and a passion for building trusting, collaborative relationships with patients and colleagues.
- Ability to adapt to a fast-paced, innovative healthcare environment, while maintaining a focus on compassionate, patient-centered care.
