About the role
The ILS (Intensive Long-Term Support) Program is an adult supportive residential facility providing 24-hour residential care with treatment and rehabilitation components. Services include psychiatric services, nursing services, case management, wrap-around and recovery services, transportation, and personal care.
Responsibilities
- Works in a multi-disciplinary team approach to meet clinical needs.
- Completes all documentation in compliance with CARF and funder standards.
- Provides linkage and/or referral to primary care physicians for treatment of medical needs.
- Demonstrates and maintains a positive work climate and the overall team effort of the department.
- Ensures that program productivity targets are met or exceeded.
- Completes a variety of reporting.
- Works to maintain a positive public image.
Qualifications
- Experience: 1-year experience required; experience with SPMI population preferred.
- Education/Knowledge: LPN degree with a current Tennessee license; knowledge base in a health-related field/nursing field; course work or experience in cultural diversity, mental illness, alcohol and drug abuse, abuse, suicide, and mental disabilities.
- Physical/Emotional/Social Skills: Exposure to biological hazards; hearing of normal and soft tones; close eye work; valid driver's license- "F" endorsement; lifting up to 50 lbs.; pushing/pulling up to 150 lbs.; certification in and adequate implementation of verbal and physical de-escalation techniques; frequent sitting, standing, walking, bending, stooping, and reaching; CPR and First Aid certification required.
