Responsibilities
- Provide day-to-day supervision, coaching, and support to ICM staff
- Conduct and document weekly treatment team meetings, ensuring each enrolled individual is reviewed at least monthly
- Monitor staff productivity, documentation quality, service delivery, and compliance with program standards
- Assist with onboarding, mentoring, and ongoing development of ICM staff
- Collaborate with leadership to identify operational needs, barriers to service delivery, and opportunities for continuous improvement
- Maintain oversight of compliance with all DBHDD Intensive Case Management standards, including required face-to-face contacts, community-based service delivery, documentation timelines, and billing requirements
- Ensure all required assessments, housing documentation, treatment plans, crisis plans, and service authorizations remain current and compliant
- Monitor engagement efforts and ensure assertive outreach strategies are utilized for difficult-to-engage individuals
- Review and address gaps in documentation, service delivery, or compliance to reduce audit findings and improve program outcomes
- Oversee coordination of behavioral health, medical, housing, financial, vocational, and community-based services
- Coordinate care transitions for individuals discharging from psychiatric hospitals, crisis stabilization units, correctional facilities, or episodes of homelessness
- Promote continuity of care through collaboration with providers, family members (when appropriate), community agencies, and natural supports
- Ensure services are delivered in the individual's preferred environment whenever possible, including homes, community locations, shelters, healthcare facilities, or other appropriate settings
- Promote person-centered, recovery-oriented services while protecting each individual's privacy and confidentiality during community interactions
- Support staff in utilizing creative engagement strategies to improve retention and reduce unnecessary discharges
- Identify trends and recommend process improvements to improve service delivery and program effectiveness
- Participate in internal audits, quality improvement initiatives, accreditation activities, and corrective action planning as needed
- Partner with internal departments, community providers, hospitals, law enforcement, housing agencies, and other stakeholders to ensure coordinated, comprehensive care
- Participate in interdisciplinary meetings and represent the ICM program during internal and external collaborations
- Maintain knowledge of DBHDD standards, Medicaid requirements, and organizational policies governing Intensive Case Management services
- Perform all other duties as assigned
Requirements
- Master's degree from an accredited college/university in the field of psychology, social work, counseling education, or human services field
- Minimum of four (4) years of documented experience working with adults with a SPMI
- Active GA counseling license (LPC, LCSW or LMFT)
- Valid Georgia Driver’s License
- Effective verbal and written communication skills
- Strong interpersonal skills and the ability to work effectively with diverse communities
- Ability to work independently and in collaboration with others
- Experience with Microsoft 365 Office Products
