For consideration you must reside within a commutable distance of Northern VA, you will have the flexibility to work remotely* as you take on some tough challenges.

Responsibilities

• The Care Coordinator will be the primary care manager for a panel of serious mental illness/SMI members with varying risk and may be assigned other health plan populations as needed.
• This position is responsible for completing Health Risk Assessments, Individualized Service Plans and linking members to needed services to ensure that the members are receiving the healthcare they need.
• Members worked within this position are those with behavioral health needs.
• This position would involve working with physicians, behavioral health providers, and other community stakeholders as well.
• Care Managers working with this population will be required to work in the field to complete face-to-face visits for 25-50% of assigned members.
• Care coordination activities will focus on supporting members' medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved
