About the role

The HS/PRS Case Manager is responsible for managing the case files of high-risk patients in the Southeast region of Florida. This role involves coordinating care plans, ensuring compliance with regulatory standards, and providing support to healthcare providers.

Responsibilities

• Manage and update patient case files

Requirements

• Bachelor's degree in Healthcare Administration, Public Health, or related field
• At least 2 years of relevant experience in healthcare management or case management
• Strong organizational and communication skills
• Proficiency in Microsoft Office Suite
