Housing Support Practitioner - Section 811

Starting salary range: $57,437 plus an office benefit package (parking/remote expenses) of $180/month, subject to change.

About the role

This position provides professional, quality program services to individuals served by the Section 811 Program. The Mental Health Practitioner will deliver Housing Support Services to people with serious mental illness and/or substance use disorders enrolled in the Section 811 program in the Metro area. The primary function is to provide outreach, housing transition, and short-term housing sustaining services, with the goal of connecting individuals to ongoing services.

This position works onsite at the RADIAS Health downtown Saint Paul office.

Schedule

Monday - Friday, 8 am - 4 pm or 9 am - 5 pm, 40 hours per week.

Responsibilities

• Engage in coordinated outreach efforts with RADIAS Health PATH team members to build connections and establish rapport with individuals once approved for Section 811.
• Assist in finding resources for household furnishings and supplies for clients who are recently housed or in need. Ensure the person served participates in the selection of household furnishings and supplies.
• Provide tenancy supports to HUD Section 811 Project Based program participants experiencing or who have experienced long-term homelessness and serious mental illness in the seven-county metro area. Assist with property applications and verifications during move-in and recertification.
• Provide instruction and monitoring on identified service areas and goals indicated in assessments, including the Treatment Plan.
• Maintain records of progress for each face-to-face meeting and telephone contact with clients and collateral contacts.
• Decide on appropriate interventions or assist the client in obtaining and maintaining the highest level of independence possible.
• Ensure standards are met according to State and County guidelines and submit appropriate reporting to Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other agencies/entities when necessary.
• Provide post-transition services to assist program participants in sustaining housing and living successfully in the community, including making referrals for long-term support services such as Targeted Case Management, ARMHS, CADI, and ongoing HSS.
• Work with the person served and the Property Manager, if needed, to solve identified problems collaboratively.
• Provide problem mitigation services for 811 participants at risk of losing housing.
• Coordinate and cooperate with other agency and community providers to ensure the best possible care for individuals, including landlords and community resources. Complete all necessary Release of Information documents and keep them current.
• Assist clients in creating a plan to work toward service goals in regular meetings, adjusting the Treatment Plan as needed.
• Schedule appointments to see clients to assist in requested services for time and frequency agreed upon in the person's support plan.
• Ensure persons served have access to information about tenant rights and responsibilities and provide ongoing guidance on adhering to lease terms.
• Transport clients in the community as needed.
• Complete all paperwork, reports, and charting contemporaneously and in an organized manner. All documentation must be completed within 24 hours of a visit per agency guidelines.
• Attend weekly meetings with DHS, AEON, staff meetings, and other scheduled meetings.
• Participate actively in ongoing professional growth and development; maintain appropriate professional behavior and participate in supervision.

Requirements

• Bachelor’s degree in behavioral science or related field and 2,000 hours of supervised experience in the delivery of clinical services in the treatment of adults with mental illness (supervised, but not necessarily by a mental health professional); or
• Completed 6,000 hours of supervised experience in the delivery of clinical services in the treatment of mental illness.

Preferred Qualifications

• Bachelor’s degree in behavioral sciences or related fields.
• Experience providing outreach services to persons experiencing homelessness and knowledge of services/resources in Ramsey County.
