The Hospital Social Worker Trainee position at MultiCare is an entry-level opportunity responsible for cross continuum care coordination, psychosocial assessments, and interventions. This role aims to improve patient care, services, and population health.
Responsibilities
- Perform cross continuum care activities in clinics, provider practices, urgent care, phone care agencies, skilled care, or other sites as needed
- Provide customer service to patients, families, physicians, APPs, and care teams, including discharge milestones and progression to next level of care
- Model and orchestrate consistent, professional communications and maintain proactive communication patterns with colleagues and staff
- Delegate responsibilities appropriately while remaining accountable for outcomes
- Ensure medically appropriate transitions of care
Requirements
- Master’s degree in Social Work with Healthcare-based practicum
- Licensed in the state of Washington as Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years
- Hospital or equivalent experience preferred; experience prior to obtaining a Master’s degree may be considered
- Health industry knowledge preferred
- Healthcare financial assistance knowledge preferred
