Hospital Durable Medical Equipment Liaison

About the role

Act as the main point of contact for hospital staff discharging patients home with Durable Medical Equipment (DME) and home oxygen. Facilitate timely delivery of DME to patients prior to discharge by communicating with Care Coordination and hospital staff within Cerner, HC+, and other Electronic Medical Record platforms. Coordinate with outside DME companies for necessary paperwork and delivery. Collaborate with Customer Service to ensure timely DME delivery, reducing readmissions and promoting seamless patient discharge. Manage inventory of the hospital DME closet.

Responsibilities

• Coordinate with outside DME companies for paperwork and delivery as needed
• Collaborate with Customer Service to ensure timely DME delivery
• Manage inventory of hospital DME closet

Requirements

• High School Graduate or equivalent GED
• 1 year in a Health Care Environment or 2 years Customer Service or 6 months Cerner and HC+ experience
• Associate Degree may substitute for experience
• Valid Florida Driver’s License
• Level II screening through the Florida Care Provider Background Screening Clearinghouse (managed by AHCA)

Schedule

• 8:00 a.m. – 4:30 p.m. or 8:30 a.m. – 5:00 p.m.
• Occasional weekends
• No on-call requirement
