Hospital Dialysis Licensed Practical Nurse

About the role

DaVita is seeking a Licensed Practical Nurse to join our team at Prisma Hospital. The position offers the opportunity to deliver care in an empathetic and compassionate manner, supporting patients and their families dealing with end-stage renal failure or chronic kidney disease.

Responsibilities

• Deliver care to patients with multiple co-morbidities requiring unique treatment plans.
• Leverage a broad range of nursing skills and knowledge to support patients in an outpatient setting.
• Work in a fast-paced environment, balancing care with empathy and professionalism.
• Build and maintain meaningful relationships with patients and their families.
• Participate in a supportive team environment, committed to improving patient health through clinical goal setting and quality improvement initiatives.

Requirements

• Current LPN-LVN license in the state of practice required.
• Current Dialysis Experience Required.
• CPR certification required.
• IV certification required if required by state regulations.
• Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system.
• BONENT CCHT or CHT certification preferred or as required by state regulation.
• Minimum of one year's experience in a hospital or clinical setting preferred.
• Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of DaVita within 90 days of hire required.

Qualifications

• High school diploma or equivalent required.
• Successful completion of the pre-employment color blind test, accommodation may be provided.

Schedule

The position requires a flexible schedule and availability to work mornings, evenings, weekends, and holidays.
