Hospital at Home Acute Care/MACU RN Team Lead

About the role

Nascentia Health is seeking a Hospital at Home (H@H) Acute Care/Mobile Acute Care Unit (MACU) RN Team Leader to assist with reviewing patients in the EMR alongside the H@H case manager and social worker for program eligibility. The MACU RN Team Leader coordinates homecare services, delivers care, and documents patient care in alignment with Quality Standards under clinical supervision.

Responsibilities

• Ensure quality and safe delivery of acute hospital care at home services.
• Assist with screening hospital patients for program eligibility.
• Schedule patients to MACU nurses and therapists.
• Educate patients and families about the program and expectations.
• Facilitate communication among departments, agencies, and an interdisciplinary team of clinicians.
• Coordinate internal and external resources to meet patient needs.
• Provide clinical care as needed to assigned patients in the private home environment, including a minimum of 2 nursing visits per day, telehealth monitoring, virtual visits, and joint visits with medical providers.
• Collaborate with medical providers at hospitals, communicate changes in patient condition, routine clinical updates, and progress toward goals.
• Administer medications and care to patients as ordered.
• Maintain skill competency for delivering quality complex care, including proficient assessment skills, high-tech IV skills, procedural skills, and equipment use (e.g., catheter care, drainage tubes, oxygen, durable medical equipment, telemonitoring).
• Perform all other duties as assigned.

Requirements

• Current license as a Registered Nurse in New York State.
• Bachelor’s degree from an approved school (preferred).
• Experience as a clinician with home and community-based services (preferred).
• Ability to provide care to individuals in private homes.
• Strong written and verbal communication skills, including conflict resolution in stressful situations.
• Proficiency in Microsoft Office or equivalent software for word processing, spreadsheets, and database management.
• Excellent organizational skills and ability to prioritize multiple tasks.
• Efficient and flexible problem-solving skills, with the ability to motivate and educate others.

Physical Requirements

• Speaking, visual, and hearing ability sufficient to comprehend and express written/verbal communication.
• Ability to perform tasks involving physical activity, including:
  • Pushing/pulling: typical weight 50 pounds, maximum weight 150 pounds.
  • Lifting/carrying: typical weight 25 pounds, maximum weight 40 pounds.
  • Patient transfers, frequent bending, standing, kneeling, walking in various weather conditions (e.g., snow, rain, sleet), and going up and down stairs.
• Ability to drive in all-weather conditions throughout the working year.
• Bloodborne Pathogens Exposure Category I (routine exposure or potential exposure to blood, body fluids, or tissues).

Pay

$82,435–$95,000 annually (S04 exempt position).
