Who We're Seeking

We're seeking a dedicated Registered Nurse to deliver personalized, one-on-one care in the comforting setting of patients' homes. Regular hours for this position are 2 - 4 days per week – a combination of Friday-Monday 8AM-5PM and acts as after hours on-call triage and runner.

Your Responsibilities

• Provide compassionate end-of-life care, ensuring the physical and emotional well-being of patients and their families.
• Deliver expert pain and symptom management, offering comfort and support during challenging times.
• Collaborate closely with our interdisciplinary team, local physicians, and community providers to optimize the quality of life for patients.

Requirements

• VT Licensed RN (BSN preferred)
• Minimum of two years of clinical med-surg and acute care nursing experience required. One year of Hospice experience is preferred.
• Home Health experience and/or previous case management experience preferred.
• Valid driver’s license
