Hospice Visit Licensed Practical Nurse - (LPN )

About the role

The Licensed Practical Nurse (LPN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and Assisted Living Facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Responsibilities

• Provide care and services for a group of patients in private homes, Skilled Nursing Facilities, and Assisted Living Facilities
• Collaborate daily with a team of clinicians as a contributing member of the interdisciplinary team
• Provide expertise regarding psych-social assessments, interventions, and knowledge of community sources
• Work closely with patients on a 1-1 level with strong support from a patient care team
• Manage visits, documentation, and meetings, including scheduled visits, required meetings, and urgent needs by prioritizing and triaging multiple concerns
• Learn and utilize an electronic medical records system

Requirements

• Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice
• One year of experience under license
• Ability to work well within a team environment
• Ability to work independently on a day-to-day basis with activities including phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of state and federal rules and regulations
• Time management skills to manage visits, documentation, and meetings
• Must be willing to drive and have reliable transportation, valid driver's license, and auto insurance
