Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups to promote Enhabit’s services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative, providing education related to Enhabit’s services.

Qualifications

Education and experience, essential

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year of experience in home care.
• Demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Possess a valid state driver license.
• Maintain automobile liability insurance as required by law.
• Maintain dependable transportation in good working condition.
• Able to safely drive an automobile in all types of weather conditions.
