Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups to promote Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative, educating stakeholders about provided services.

Qualifications

Education and experience, essential

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year of experience in home care.
• Demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Possess a valid state driver license.
• Maintain automobile liability insurance as required by law.
• Maintain dependable transportation in good working condition.
• Able to safely drive an automobile in all types of weather conditions.
