About the role

AccentCare is seeking a dedicated and collaborative Hospice Team Physician to provide clinical leadership and medical expertise as part of an interdisciplinary hospice care team. This physician will play a vital role in supporting patients with terminal illnesses, partnering with attending physicians, and ensuring the delivery of high-quality, patient-centered hospice care. This position offers the opportunity to work closely with nurses, social workers, chaplains, and other healthcare professionals while making a meaningful impact on patients and families during critical moments in their healthcare journey.

Responsibilities

  • Participate in Interdisciplinary Group (IDG) meetings and care planning.
  • Evaluate, certify, and recertify hospice eligibility.
  • Provide expertise in pain and symptom management.
  • Collaborate with attending physicians, nurses, social workers, and chaplains.
  • Review medications and prescribe treatments as appropriate.
  • Participate in an on-call rotation providing 24/7 physician coverage.
  • Support hospice admissions, discharges, and care transitions.
  • Complete timely and accurate clinical documentation.

Qualifications

  • Maintains active medical and DEA license in the state of Texas
  • Knowledge of primary medical care principles.
  • Working knowledge of hospice and palliative care.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively within an interdisciplinary care model.

Preferred qualifications

  • Five or more years of clinical experience
  • Oversight of clinical program(s)
