Hospice Social Worker - (PRN) Council Bluffs, IA

About the role

Care Initiatives is currently seeking a PRN Hospice Social Worker to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities.

Key Responsibilities

• Assess patients’ psychosocial needs and complete social histories.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support patients and families with adjustment, rights, and orientation.
• Coordinate referrals and act as a liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support patient, family, and community engagement activities.
• Promote facility programs and services to referral partners.

What it takes to be a Social Worker at Care Initiatives Hospice

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and current, valid social work license (LMSW).
• Hospice or palliative care experience, preferred.
• Two (2) years of verifiable experience working as a Social Worker, preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
