About the role
Care Initiatives is currently seeking a PRN Hospice Social Worker to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities.
Key Responsibilities
- Assess patients’ psychosocial needs and complete social histories.
- Document care plans, progress, and follow-up per regulations.
- Provide counseling and emotional support to residents and families.
- Support patients and families with adjustment, rights, and orientation.
- Coordinate referrals and act as a liaison with community resources.
- Partner with the care team to enhance quality of life and outcomes.
- Plan and support patient, family, and community engagement activities.
- Promote facility programs and services to referral partners.
What it takes to be a Social Worker at Care Initiatives Hospice
- Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and current, valid social work license (LMSW).
- Hospice or palliative care experience, preferred.
- Two (2) years of verifiable experience working as a Social Worker, preferred.
- Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
