Hospice Social Worker (BSW or MSW)

About the role

We are looking for an experienced Social Worker to work as part of our hospice interdisciplinary team serving the Dexter, MO area. The ideal candidate would have some medical care experience and live within a commutable distance to the Dexter office.

Responsibilities

• Visit patients wherever they reside—private home, skilled nursing facility, assisted living facility, or hospital.
• Complete a psychosocial assessment on each assigned patient in a timely manner and document in the electronic medical record.
• Utilize community resources for patients, such as in-home services, Meals on Wheels, etc.
• Assist the patient, family, and caregiver in coping with issues related to death and dying and participate in grief support as needed.
• Provide information, complete hospice admission paperwork, and educate patients, families, and the community on all aspects of hospice services.

Requirements

• Graduated from a School of Social Work approved by the Council of Social Work Education with a Master's or Bachelor's Degree in Social Work. Other related degrees/areas of study will not be considered as they are not allowed by state regulations.
• Computer skills for charting/EMR, basic use of Word, Excel, and Email; smart phone for mileage apps, etc. Willing to train the right person.
• A loving, compassionate heart is a must.

Schedule

Work hours are primarily Monday through Friday, 8am to 5pm, with the understanding a crisis can occur at any time. Flexibility with scheduling and hours is required.

Pay

Hourly rate with mileage reimbursement provided. Compensation is based on years of experience.
