Hospice RN (PRN) - On Call Team

Hospice of Marion, a Part of Empath Health, is seeking an RN (PRN) to join our On Call team to visit patients and provide clinical, physical, educational and emotional support after-hours in patient homes, long-term care facilities, and/or assisted living facilities in Marion County.

Schedule

• Day shifts and overnight shifts available
• 2 weekend shifts per month (12-hour shifts)

Responsibilities

• Demonstrate the ability to obtain patient history, perform an age-specific physical, psychosocial, and spiritual assessment, and initiate appropriate referral, intervention, and collaboration between the patient, family, Interdisciplinary team (IDT), physician, and partnering facility (as applicable) following policy and practice guidelines.
• Identify age-specific patient and caregiver learning needs, motivations, barriers, abilities, and preferences, and utilize teaching strategies/instructions with follow-through.
• Ensure documentation complies with all organizational standards, including productivity, timeliness, and adherence to relevant policies and practice guidelines.
• Following a patient’s death, assist with closure contact and support in collaboration with the IDT.
• Direct and supervise the care provided by the LPN and Home Health Aide following agency policy. Evaluate services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.
• Educate and represent the community regarding hospice care philosophy and concept through case-by-case consultations, educational in-services, or seminars as assigned.
• Provide education to IDT members regarding nursing aspects of patient/family care.
• Demonstrate the ability to administer, monitor, and document medications as prescribed by the attending physician. Use age-specific care practices and provide appropriate patient/family teaching.

Requirements

• Associate’s degree (A.A.) or equivalent from a two-year college or technical school
• Minimum of one year of related experience and/or training; or equivalent combination of education and experience
• Valid Florida license as a Registered Nurse
• Valid Florida State Driver’s License
• Reliable transportation and valid automobile insurance (for field-based positions)

Preferred Qualifications

• One (1) year of experience in an acute or post-acute care setting
• Experience caring for terminal patients and others with life-threatening illnesses
