Hospice RN Nurse Case Manager

About the role

We are seeking a Registered Nurse to serve patients throughout the Sumter, SC area as a Nurse Case Manager. This role is a critical part of the patient’s care team, visiting patients in their homes, long-term care facilities, and other settings to evaluate needs and initiate telemedicine visits. Travel is required daily, and reliable transportation is mandatory.

Schedule

  • Track Schedule 7 on 7 off Pittman Schedule (2 on, 2 off, 3 on)
  • 8-hour shifts Monday–Friday (8:00 am–5:00 pm)

Responsibilities

  • Facilitate receiving and review of all medical records from the patient’s primary provider and specialists.
  • Complete consents with patients and enroll eligible patients in Care Management.
  • Initiate Care Management Plan of Care for eligible patients.
  • Capture all diagnoses at the highest specificity by creating gaps and ensuring acceptance.
  • Complete Annual Wellness Visits (AWVs) for provider review.
  • Perform cognitive impairment screenings and Social Determinants of Health (SDoH) assessments.
  • Complete Advance Care Planning (ACP) discussions for provider review.
  • Evaluate patients for home health, hospice, palliative care, or specialty consults.
  • Evaluate for Remote Patient Monitoring (RPM) devices and tools to improve quality of life.
  • Reconcile prescribed and over-the-counter medications, vitamins, supplements, herbal remedies, and other treatments.
  • Provide post-discharge education and evaluate for adaptive equipment and Durable Medical Equipment (DME).
  • Assess for safe environment and evaluate acute conditions or follow-up needs.
  • Accurately document and log Care Management activities.
  • Work with the care team to keep patient Care Management plans up to date.
  • Coordinate with healthcare providers, therapists, home health or hospice representatives, and others involved in the patient’s care plan.
  • Facilitate visits with appropriate providers or entities and coordinate telehealth visits when necessary.
  • Participate in Interdisciplinary Group (IDG) meetings as needed.

Qualifications

  • Must be a Registered Nurse with an active, in-good-standing license.
  • A minimum of three (3) years of clinical experience preferred.
  • Experience in community settings preferred.
  • Proven ability to communicate and collaborate effectively with interdisciplinary care teams, patients, and caregivers.
  • Strong written and verbal communication skills and basic computer proficiency.
  • Ability to manage multiple responsibilities, demonstrate sound judgment, and foster a cooperative work environment.
  • Must be able to work flexible hours and travel between offices and facilities.
  • Must be a licensed driver with an insured vehicle in good working order.
