About the Role

We are looking for an RN Field Staff - Hospice who is passionate about making a difference in the lives of those needing healthcare or embracing the next chapter of their lives. In this role, you will be responsible for the planning, coordination, and delivery of quality hospice care and services, ensuring compliance with company standards and federal, state, and local regulations.

Responsibilities

  • Provides ongoing assessment and nursing care to patients in homes and healthcare facilities.
  • Ensures an accurate evaluation of patient needs and develops a comprehensive care plan addressing current needs.
  • Follows current nursing professional standards of practice and agency-approved protocols for hospice.
  • Provides effective pain and symptom management for patient comfort.
  • Communicates with other healthcare providers about changes in patient condition and needs.
  • Assesses patient and caregiver needs and provides education when necessary.
  • Commits to high-quality customer service for internal and external customers.
  • Prepares for IDG by reviewing the Care Plan, reconciling medications, and projecting patient/family needs.
  • Audits and maintains current and accurate patient records through electronic and paper documentation.
  • Participates in quality improvement projects as needed.
  • Maintains knowledge of regulatory agency requirements, accrediting bodies, and third-party payers.

Qualifications

  • Current unencumbered Ohio RN license.
  • One year of experience in acute care or medical-surgical nursing.
  • Experience in a Medicare-certified home health agency preferred.
  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
  • Proficiency with electronic medical records (EMR) preferred.

Requirements

  • Must be able to read, write, speak, and understand English.
  • Must possess a valid driver’s license and acceptable driving record.
  • Must be insurable under professional liability and crime coverage policies.

Working Conditions

  • Sitting: Up to 4 hours/day
  • Standing: Up to 8 hours/day
  • Walking: Up to 6 hours/day
  • Lifting, transferring, pushing, or pulling patients or equipment/supplies: Up to 50 pounds
  • Driving: Up to 6 hours/day
  • Work weekends, evenings, and holidays as scheduled
  • On-call availability 24/7 for emergencies
  • Exposure to falls, burns from equipment, odors, and cuts during the workday
  • Occasional exposure to patients with various disease processes
  • Exposure to infectious waste, disease, and conditions, including AIDS and Hepatitis B viruses
