Hospice RN Continuity of Care and Triage

Responsibilities

• Provide clinical guidance and support to hospice field staff.
• Triage calls from patients, families, facilities, and staff.
• Cookordination visits, staffing assignments, and resources.
• Escalate high-risk situations as needed.
• Ensure timely and accurate documentation.
• Support quality improvement initiatives and regulatory compliance.
• Educate, mentor, and support hospice team members.
• Collaborate with leadership to ensure continuity of care.

Qualifications

• Active, unrestricted RN license.
• Minimum 2 years of hospice experience required.
• On-call triage experience preferred.
• Strong clinical judgment and critical thinking skills.
• Excellent communication and organizational abilities.
• Commitment to compassionate, patient-centered end-of-life care.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $89,700 to USD $97,800.

This range is based on the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
