As a Hospice Registered Nurse Case Manager (RNCM), you will help patients and their families navigate the most intimate and difficult time of one’s life. Using hospice best-practice care delivery approaches, you will ensure an excellent end-of-life experience for your patients. You will work with an interdisciplinary team to deliver a unique experience for every patient.

What You Will Love Doing

• Symptom management: Work with the patient’s attending physician and hospice Medical Director to manage symptoms.
• Plan of Care: Evaluate and adjust the plan of care based on the patient’s condition.
• Continuity of Care: Manage interdisciplinary care and promote continuity of care.
• Family Educator/Advocate: Counsel patients and families, incorporating them in the plan of care.
• Documentation: Complete clinical documentation following agency protocol and Medicare/Federal guidelines.
• Operations: Supervise and oversee the care provided by nurse aides and LPNs.

Requirements

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting preferred.
• Hospice experience desired.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
