About the Role

Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to work within and around the Fulton / Cobb County, GA region. As a member of the interdisciplinary team, the RN Case Manager works under the direction of the Director of Nursing, the attending physician, and the interdisciplinary group to provide skilled nursing assessments, planning, and care. This role maximizes the comfort and health of patients and their loved ones or caregivers by identifying needs and delivering supportive care in accordance with the attending physician’s orders and plan of care.

Responsibilities

  • Assume patient caseload assigned by the Patient Care Coordinator or designee.
  • Take primary responsibility for a patient caseload, including assessing, planning, implementing, and evaluating phases of the nursing process.
  • Initiate communication with attending physicians, Hospice staff, and other agencies to coordinate optimal care and resource use for patients and their loved ones or caregivers.
  • Maintain regular communication with the attending physician concerning patient and caregiver needs.
  • Seek input from other team members to enhance the patient plan of care.
  • Provide accurate and timely documentation in electronic medical records.
  • Participate in and contribute to Interdisciplinary Group meetings.
  • Coordinate physical care by teaching primary caregivers, volunteers, or employed caregivers, or by providing direct care as needed.
  • Meet regularly with Hospice nursing staff to review caseload issues, share professional support, and exchange feedback for growth.
  • Attend patient care conferences and facilitate discussion of caseload issues for staff consultation and evaluation.
  • Supervise and document the work of Nursing Assistants (NAC) every two weeks or as assigned.
  • Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues.
  • Serve as an associate nurse for patients of other primary nurses during off-hours or as requested.
  • Provide support at the time of death and during the bereavement period.
  • Participate in Hospice orientation and in-service training programs for professional staff.
  • Engage in agency and community programs to promote professional growth and understanding of hospice care.
  • Maintain familiarity with agency policies and state/federal regulations governing hospice care.
  • Accept other assignments as appropriate.

Requirements

  • Graduate from an accredited School of Nursing.
  • Current Registered Nurse (RN) license under the provisions of Article 1 of Chapter 26 of Title 43 of the Official Code of Georgia.
  • One year of RN experience; hospice experience preferred.
  • Excellent assessment and problem-solving skills.
  • CPR certified for patient care.
  • Strong organizational skills and knowledge of time management and case management processes.
  • Licensed driver with an insured automobile in good working order.
  • Strong verbal and written communication skills.
  • No exceptions to RN license; must be able to administer pharmaceuticals, perform medication reconciliation, and conduct nursing procedures within the scope of practice.
  • All skills must be deemed competent before hands-on patient care, especially for infrequently performed procedures or treatments.
