Hospice RN Case Manager

About the role

Hospice nursing offers autonomy and independence, allowing you to work in the field visiting patients in various facilities and homes. This role provides the opportunity to make a meaningful difference for patients facing terminal illness and their loved ones.

We are seeking a Full Time RN Case Manager to serve patients in the Ozark, MO area. Part time or PRN positions may also be available. The role involves driving to patient locations within a set geographical area, with occasional required meetings at the office.

Responsibilities

• Perform head-to-toe assessments and provide symptom management and skilled care to patients facing terminal illnesses.
• Offer emotional support and education on death and dying to patients and their families.
• Supervise LPNs and CNAs.
• Monitor medication lists and assess the appropriateness of medications.
• Evaluate and address durable medical equipment (DME) needs.
• Provide wound care and other necessary treatments.
• Visit patients in private homes, assisted living facilities, skilled nursing facilities, and other locations where patients reside.

Requirements

• Current RN license in the State of Missouri.
• Must live within a commutable distance to the office.
• A gentle, loving heart and the ability to provide compassionate support to patients and families facing terminal illness.
• Ability to work as a team player and represent the company well in the community.
• Computer and smartphone skills for EMR charting, email, and mileage computation apps.
• Reliable transportation and car insurance.

Pay

Hourly rate plus mileage reimbursement.

Schedule

The offices are open Monday through Friday, 8am to 5pm.
