About the Role
JourneyCare Hospice is seeking a Registered Nurse (RN) Case Manager to join our care team in Woodstock, IL. This full-time position offers a Monday-Friday schedule from 8:30 AM to 5:00 PM, with on-call duties every 12th weekend (Saturday and Sunday).
Responsibilities
- Assume primary responsibility for patient and family caseload, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.
- Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs; initiate intervention by other members of the interdisciplinary team.
- Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
- Provide regular visits to re-assess the patient's needs and update the plan of care.
- Perform clinical pronouncement of death and provide support to the family.
- Maintain accurate patient records, charts, progress notes, reports, and prepare care plans in a timely manner.
- Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines.
Requirements
- Graduate from an accredited registered nursing program.
- Licensed as a registered nurse in the state of practice.
- Current CPR and First Aid Certification.
- Knowledgeable of quality assessment and assurance procedures.
- Valid driver’s license and proof of insurance.
