About the role
We are seeking a dedicated and compassionate Hospice RN Case Manager to join our team in the North Easton area. Full-time and part-time openings available. Weekly pay up to $51 an hour, depending on experience.
Responsibilities
- Supervise hospice aides
- Complete comprehensive assessments of patient and family to determine hospice needs
- Provide professional nursing care by utilizing all elements of the nursing process
- Develop and implement individualized care plans
- Collaborate with interdisciplinary team members to ensure comprehensive care
- Provide health care instructions to patients and families
- Counsel patients and families on meeting nursing and related needs
Requirements
- Graduate of an accredited school of nursing
- Must possess an active and unencumbered RN license in Massachusetts
- Prior hospice experience preferred
- CPR certified
