Responsibilities
- Supervise hospice aides
- Conduct comprehensive assessments of patients and families to determine hospice needs
- Develop and implement individualized plans of care
- Cookordinate patient care services
- Provide professional nursing care and counsel patients and families
- Participate in ongoing education and quality improvement initiatives
Requirements
- Graduate of an accredited school of nursing
- One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care
- Current licensure in the state and CPR certification
- Bachelor's degree, with one year of hospice care experience preferred
