About the Role
We are seeking an experienced and compassionate RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families. This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel throughout Plymouth County and portions of the Cape.
Responsibilities
- Supervise hospice aides
- Complete comprehensive assessments of patient and family to determine hospice needs
- Provide professional nursing care by utilizing all elements of the nursing process
- Develop and implement individualized care plans
- Collaborate with interdisciplinary team members to ensure comprehensive care
- Provide health care instructions to patients and families
- Counsel patients and families on meeting nursing and related needs
Requirements
- Graduate of an accredited school of nursing
- Must possess an active and unencumbered RN license in Massachusetts
- Prior hospice experience preferred
- CPR certified
Pay
Up to $51 an hour, depending on experience, with weekly pay.
