About the role

Full-time, part-time, and per diem RN Case Manager positions available in Berkshire County. This is an on-the-road role requiring travel and a reliable, insured vehicle.

Pay

$48 to $50 an hour, paid weekly.

Responsibilities

  • Supervise hospice aides.
  • Complete initial, comprehensive, and ongoing assessments of patient and family hospice needs.
  • Provide a complete physical assessment and history of current and previous illnesses.
  • Deliver professional nursing care using all elements of the nursing process.
  • Assess and evaluate patient status; write and initiate a plan of care, regularly re-evaluate needs, and revise the plan as necessary.
  • Initiate the plan of care and make revisions as patient status and needs change.
  • Use health assessment data to determine nursing diagnosis.
  • Develop a care plan with goals based on nursing diagnosis, incorporating palliative nursing actions and including the patient and family in planning.
  • Initiate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by the physician in the plan of care.
  • Counsel the patient and family on meeting nursing and related needs.
  • Provide health care instructions to the patient as appropriate per assessment and plan.
  • Assist the patient with activities of daily living and facilitate efforts toward self-sufficiency and optimal comfort care.
  • Act as Case Manager when assigned by Clinical Supervisor, coordinating patient care for an assigned caseload.

Requirements

  • Graduate of an accredited school of nursing (bachelor’s degree preferred).
  • Active and unencumbered RN license in Massachusetts.
  • One to two years of recent acute care experience in an institutional setting.
  • One to two years of recent home care experience (preferred).
  • CPR certification.
  • Up-to-date COVID and flu vaccinations or acknowledgment of declination.
