About the role
Full-time, part-time, and per diem RN Case Manager positions available in Berkshire County. This is an on-the-road role requiring travel and a reliable, insured vehicle.
Pay
$48 to $50 an hour, paid weekly.
Responsibilities
- Supervise hospice aides.
- Complete initial, comprehensive, and ongoing assessments of patient and family hospice needs.
- Provide a complete physical assessment and history of current and previous illnesses.
- Deliver professional nursing care using all elements of the nursing process.
- Assess and evaluate patient status; write and initiate a plan of care, regularly re-evaluate needs, and revise the plan as necessary.
- Initiate the plan of care and make revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan with goals based on nursing diagnosis, incorporating palliative nursing actions and including the patient and family in planning.
- Initiate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by the physician in the plan of care.
- Counsel the patient and family on meeting nursing and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Assist the patient with activities of daily living and facilitate efforts toward self-sufficiency and optimal comfort care.
- Act as Case Manager when assigned by Clinical Supervisor, coordinating patient care for an assigned caseload.
Requirements
- Graduate of an accredited school of nursing (bachelor’s degree preferred).
- Active and unencumbered RN license in Massachusetts.
- One to two years of recent acute care experience in an institutional setting.
- One to two years of recent home care experience (preferred).
- CPR certification.
- Up-to-date COVID and flu vaccinations or acknowledgment of declination.
