Hospice RN Case Manager
Join our team at Hospice of Western & Central Massachusetts and make a lasting difference in the lives of patients and families! We are seeking a compassionate and experienced Hospice RN Case Manager to join our team in Berkshire County.
About the role
As a Hospice RN Case Manager, you will be responsible for providing high-quality, patient-centered care to individuals and families in their homes and communities. This is an on-the-road position requiring the candidate to have reliable, insured transportation.
Pay: $48 to $50 an hour with weekly pay.
Responsibilities
- Plan, organize, and direct hospice care, building from the resources of the community to meet the needs of patients and families
- Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
- Provide professional nursing care by utilizing all elements of the nursing process
- Develop and implement individualized care plans, addressing physical, emotional, social, and spiritual needs
- Collaborate with patients, families, and healthcare providers to provide comprehensive care
- Supervise hospice aides and act as Case Manager when assigned by Clinical Supervisor
Requirements
- Graduate of an accredited school of nursing, bachelor's preferred
- Must possess an active and unencumbered RN license in Massachusetts
- One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care, preferred
- CPR certification
