About the role
Seeking a full-time Hospice RN Case Manager to provide skilled nursing assessments, care planning, and support to patients with a focus on comfort and quality of life.
Responsibilities
- Managing a patient caseload
- Documenting in EMR
- Coordinating with physicians and staff
- Educating caregivers
- Supervising nursing assistants
- Participating in team discussions and training
- Responding to patient needs during off-hours
- Supporting families at end-of-life
- Ensuring compliance with policies and regulations
Qualifications
- Valid RN license in Pennsylvania
- 1-2+ years nursing experience, preferably in hospice care
- Valid driver’s license, reliable vehicle, and good driving record
Skills
- Strong assessment, documentation, and communication skills
- Ability to work independently and collaboratively within an interdisciplinary team
- Empathy for patients and families, emotional resilience, and professionalism
Pay
Salary starting at $90,000
