What to Expect in This Role
Extensive Travel & Home-Based Care: Be prepared to travel several hours per day, covering multiple locations (1-10 home visits daily).
Deliver Compassionate, Hands-On Care: Work in diverse and unpredictable home environments, utilizing a tablet or laptop for real-time medical record documentation during each patient visit.
Management & Decision-Making: Apply clinical expertise to make sound decisions that impact patient care, solving moderately complex problems using experience, precedent, and sound judgment.
Collaborate Effectively: Collaborate effectively with interdisciplinary team members to improve patient outcomes.
Qualifications & Requirements
- Education & Experience: Bachelor’s degree (or higher) in a health-related field, minimum of 3 years of clinical RN experience (hospice experience required), at least 1 year of supervisory or management experience in a hospice setting, strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards (JCAHO knowledge preferred).
- Licenses & Certifications: Valid RN license in the state of practice, Certified Hospice and Palliative Care Nurse (CHPN), Active BLS certification (American Heart Association or Red Cross), valid driver’s license and auto liability insurance for home visits.
